DIFD-4 - Insurance Declaration Page

The declaration page is requested when a customer’s insurance does not verify electronically. The declaration page is reviewed to determine that the insurance policy meets Tennessee’s minimum liability coverage. Customers can receive a declaration page from their insurance company.
